射频消融治疗原发性肝细胞癌患者的MRI影365bet娱乐官网网址像变化研究

摘要:

目的 探讨原发性肝细胞癌患者射频消融治疗前和治疗后MRI成像的变化情况。方法 选取188例原发性肝细胞癌患者共190个肝癌病灶行射频消融术。患者治疗前2 d行MRI检查,扫描T1WI、T2WI、动态增加及弥散加权成像(diffusion-weighted imaging,DWI)序列,并测定治疗前、后正常部位、病灶部位和过渡部位的表观扩散系数(apparent diffusion coefficient,ADC)值进行数据分析。结果 术前MRI检查显示,T1WI均为低信号,T2WI为稍高信号;动态扫描显示为信号增高,动脉期强化明显,门静脉期强化减退。DWI均为高信号;ADC均为低信号。术后MRI检查,T1WI信号有所增高,T2WI信号有所下降;ADC图像均有所增高。DWI除27个高信号外,其余均降低。所有患者动态增强扫描均未出现动脉期强化。与治疗前比较,治疗后坏死灶和手术部位过渡区的ADC值均增高(均P<0.05)。治疗前正常部位的ADC值均较病灶部位和过渡部位大,过渡部位ADC值高于病灶部位,三者两两比较,差异均具有统计学意义(均P<0.05);术后三者ADC值比较,差异无统计学意义(P>0.05)。结论 MRI成像技术分辨率高,结合动态增强扫描、ADC和DWI技术,是评价射频消融治疗原发性肝细胞癌疗效的有效手段。

Objective To investigate the changes of MRI images in patients with primary hepatocellular carcinoma before and after radiofrequency ablation. Methods A total of 188 patients with primary hepatocellular carcinoma (HCC) were enrolled, and treated by radiofrequency ablation (190 liver cancer loci). Patients received MRI examination at two days before the operation. The T1WI, T2WI, dynamic increase and diffusion-weighted imaging (DWI) sequences were scanned. The apparent diffusion coefficient (ADC) values of normal areas, lesions and transitional areas were also measured before and after the treatment. Results Preoperative MRI showed that T1WI was low signal, T2WI was slightly higher signal. Dynamic scan showed increased signal, with obviously enhanced arterial phase and decreased enhancement at the portal vein phase. DWI was high signal, yet ADC was low signal. Postoperative MRI images suggested that the T1WI signal was increased, whereas the T2WI signal was decreased. ADC images were increased. DWI showed reduced signals except 27 high signals. None of the patients underwent dynamic contrast enhanced arterial phase enhancement. After the treatment, the ADC values of both the necrotic areas and the transitional areas were increased compared with those before the treatment (both P<0.05). Moreover, before the treatment, the ADC value of the normal area was higher than those of the lesion area and the transitional area (both P<0.05), and the transitional area had a higher ADC value than the lesion area. Nevertheless, the ADC value of the three regions were comparable after the treatment (P>0.05). Conclusion MRI imaging technology has high resolution. Combined with dynamic enhanced scanning, ADC, and DWI technologies, MRI is an effective approach to evaluate the efficacy of radiofrequency ablation in the treatment of primary hepatocellular carcinoma.
